LETTER OF AUTHORIZATION (LOA)
Use this form to submit an LOA request to Port your Existing Phone Number(s) to One40.
To transition your current telephone numbers to One40 Telecom LLC, we must work with your current service provider to ensure that your service is uninterrupted, and that the numbers you designate below are properly transferred. This document authorizes One40 Telecom LLC to request and receive from your existing provider
1) data and billing information (including Customer Proprietary Network Information);
2) a copy of your Customer Service Records (CSR) for the telephone numbers listed below including, but not limited to, any associated line features.
In signing below, you are selecting One40 Telecom LLC as the preferred provider for the following service(s) for the telephone number(s) listed below

__X__ Intra-LATA Service (Long Access Transport Area) (Long Distance Calls)
__X__ International Service (Calling Outside the United States)
__X__ Inter-LATA Service (Local Access Transport Area) (Calling outside your local long-distance area)
This letter authorizes transfer of your service and the telephone numbers listed below to One40 Telecom LLC, and any RespOrg used by One40 Telecom LLC. Service Address (Primary Address where the telephone service will be located. No Post Office Boxes)
Billing Address (If different from your service address, should appear EXACTLY as it does on your local telephone bill)

List below all Telephone Number(s) for which you authorize changes from your current service provider to One40 Telecom LLC. Please note that your telephone and/or associated service will be changed to One40 Telecom LLC.

Important: Please do not place any new service orders with your current provider on these accounts, as this will delay porting your numbers. A bill copy which is less than 30 days old is required to complete the transfer of number(s). Please include a summary copy showing company name and the numbers owned.
